How To Enroll
Step 1: Enroll online by clicking Here
Step 2: Once enrolled, the office will call to welcome you and schedule your first new patient appointment.
At that time, you will be charged a non-refundable $99 appointment hold/enrollment fee for you and your family.
Step 3: The first new patient visit consists of a 1 hr comprehensive history, review of records, and physical exam (your membership starts at the first appointment with your physician for you and any family member that enrolls with you, otherwise there will need to be separate enrollment fees applied).
Step 4: After every appointment, your physician will email a recap of your visit with your personalized assessment and treatment plan to provide quality care and restore balance in your life.
Follow-up appointments can be virtual or in office (upon request).
