How To Enroll
Step 1: Enroll online by clicking Here
Step 2:
Once enrolled, the office will call to welcome you and schedule your first new patient appointment.
At that time, you will be charged a non-refundable $99 appointment hold/enrollment fee for you or your entire family.
Step 3: The first new patient visit consists of a 1 hr comprehensive history, review of records, and physical exam (your membership starts at the first appointment for you or any family member).
Step 4: After every appointment, Dr. Singh/Dr. Porter emails a recap of your visit with your own personalized assessment and treatment plan to provide quality care and restore balance in your life.
